Provides assistance in transitional planning for participants and collaborates with therapeutic team to ensure coordination of care. Provides daily support to participants in educational, vocational, cultural, recreational and social areas as related to need and individual transitional plans. Communicates daily activities and programmatic strengths and concerns to staff and supervisor, and completes required documentation as required. As a member of the transitional team, acts as a team player and as a resource to help understand and problem solve as daily interactions and situations may at times pose barriers. Helps identify participants individual needs to plan for transitional services in life skills areas such as, but not limited to; vocational development, post-secondary education, financial management, nutrition, recreation, socialization, identity exploration, and healthy relationships. Serves as a role model, particularly regarding treating oneself and others with respect and dignity. Provides daily life skills training, helps in continued program advancement and fosters a strengths based culture. Participates in supervision with the Transitional Manager.
Transitional Services: Work in collaboration with participants to create individual transition plans. Ensure program planning enables transition goals to be achieved.
Documentation/Planning/Training: Completes documentation accurately and timely. Provides life skills training, helps in continued program advancement and fosters a strengths based culture. Participates in supervision with the Transitional Manager of the Brooke Whitted Center.
Program Maintenance: Participate in the development of policies and procedures for program and provides necessary support to other program staff. Models therapeutic and effective interactions with the participants throughout all aspects of the program. Participate in all program events and activities including, but not limited to: holidays, birthdays, graduations, new participant arrivals and anniversaries.

Education/Experience Minimum Requirements:
Bachelor's degree required, preferably with a background in education or counseling. Minimum of one year of experience in providing transitional planning. Minimum of one year providing cross coordination of care. Minimum of one year of experience providing supportive services for emerging adults.
Excellent organizational skills required.
Excellent oral and written communication skills required.
Ability to multi-task required.
Ability to problem solve and prioritize tasks.
Proficiency in Microsoft Word and Excel required.

Cultural competency required.
Physical capacity to be trained in and to utilize non-violent physical intervention techniques required.
Physical ability to monitor and supervise an active group of participants required.
Sophisticated understanding of emerging adults and transitional life changes that may occur and the ability to create and follow program planning that will enable participants to have a successful transition.
Ability to lead by example and provide supervision that helps the program and staff to grow and develop.
